A shell game in Sacramento: promises up front, costs passed on later.
California Created the Fraud Themselves: The Medi-Cal Open Door Policy
If you follow the latest press releases from the Governor Newsoms Press Office, you might believe that California magically solved the healthcare crisis while receiving nothing back from the federal government. This narrative belongs in the fiction section of the Sacramento Public Library. In reality Medi-Cal has become a giant shell game where the state makes grand promises to everyone regardless of their legal status and then waits for the rest of the country to send the cash.
The Myth of the Controlled Border
The Governor loves to jump on social media to fact check anyone pointing out the obvious insanity of this system. He recently responded to a viral post about Mexican citizens crossing the border to pretend to be illegal aliens just to grab our benefits. Newsom acts shocked that people exploit a system he literally designed for exploitation. As of January 1 2024 California expanded full scope Medi-Cal to every undocumented adult in the state. This policy essentially sent a gold embossed invitation to anyone within driving distance of the border. It is not a conspiracy theory when state policy makes immigration status irrelevant for healthcare.
The ID Loophole and Easy Residency
A popular myth suggests this expansion is a self funded triumph of California values. In truth an open door policy makes it surprisingly easy to walk through. Thanks to state laws like AB 60 anyone can secure a government issued photo ID simply by using a local address. If you can use a cousin’s mailbox in San Diego or a friend’s apartment in Sacramento you have officially established residency in the eyes of the DMV and the healthcare system. The state verification process functions as an honor system that rewards anyone with a zip code and a signature.
The Invisible Fraud Squad
The state claims they have plenty of oversight to catch this behavior but the actual 2026 numbers show the “fraud squad” is a joke.
| Agency | Role | Staffing |
| DHCS Investigations Division | Beneficiary fraud (cousin’s address) and audits | ~250–300 Sworn Investigators |
| DOJ Division of Medi-Cal Fraud | Provider fraud and criminal rings | ~200–250 Staff |
With roughly 15 million members on Medi-Cal and only about 300 investigators tasked with beneficiary fraud the math fails. You have one investigator for every 50,000 people. It is mathematically impossible to catch someone using a cousin’s address in a sea of 15 million files. The state knows this and simply does not care because they are too busy patting themselves on the back.
The Per Capita Truth
Gavin loves to brag about California being the 4th largest economy in the world and he is obsessed with the idea that we pay more in federal taxes than anyone else. While we do cut a massive check to D.C. the reality is that California receives more federal money back than any other state. We rake in over 530 billion dollars in federal spending including the massive 107 billion dollar subsidy for Medi-Cal. When you look at the math per capita California is essentially being carried by the rest of the country because our federal return per person is near the bottom. The average Californian contributes about 20,582 dollars to the federal government while the return is only 13,500 dollars per person. We are basically the richest guy in the room who still expects everyone else to chip in for his lunch.
The Federal Life Support System
While the state takes credit for this generosity the federal government keeps the lights on. D.C. provides over 100 billion dollars annually to California for Medi-Cal which accounts for roughly 65% of the entire program budget. Despite the Governor constant complaining that we are a donor state our entire social safety net would collapse tomorrow without the massive transfer of federal tax dollars. We are basically the neighbor who throws a lavish party on a shared credit card and then complains when the bill arrives.
The Looming Budget Collapse
The neighbor is finally running out of credit. California currently stares down a chronic structural deficit that analysts expect to hit between 20 billion and 35 billion dollars annually through 2029. This is the direct result of Newsom’s obsession with spending money we do not have on programs that do not work. We have dumped over 22 billion dollars into homelessness since 2019 only to see the problem get worse while audits show the state literally lost track of billions in spending.
Now that the federal H.R. 1 bill is slashing the match for emergency services from 90% to 50% the state is panicking. They are already proposing enrollment freezes and cutting dental benefits for the very people they used as political props. It turns out that being the most progressive state in the union is easy when someone else is paying the tab but it is a lot harder when you have a 30 billion dollar hole in your pocket and the bill actually comes due.
